Doctors Information

Please include the doctors full names and qualifications (e.g. Dr Andrea Green MB ChB DCH DRCOG Regd 1987)

Reception personnel

This section allows you to feature your key practice personnel (e.g. Practice Manager - Sally Johnson)

Practice opening times

Please state the opening times for your surgery (e.g. 9.00 - 11.30am and 1.30 - 6.30pm)

Repeat prescriptions

Here you can provide details on your repeat prescription procedure (e.g. Repeat prescription requests cannot be accepted over the phone except for housebound patients. Please post your Repeat Slip in the box on reception. Your prescription should be ready within 48 hours if requested during the week).

Home visits

Health clinics and promotions

Please list each clinic you wish to promote (one clinic per box). These could include Well Man/Woman Clinics, Baby clinics, Cervical smear screening and Travel Vaccinations.
